Once your payment has been made you can download the Patient Forms and Make your Appointment.
 
This package gives you two months of herbal care, which will include one 60-90 minute intake, and two 30 minute follow-up appointments. You will have the freedom to contact me (via text or email) as issues or questions arise. With this package, you will receive a wellness protocol geared towards your primary area of concern. It will include herbal, dietary, and exercise recommendations, as well as guidance on any non-herbal supplements I believe, will be a benefit to you.
